Electronic Filing of Notice of Address Change

Creditors are required to file a Notice of Address Change
electronically. A login/password is not required.

Attorney filers are required to access ECF and docket a notice of
address change electronically under Notice.

The entry of creditor information will complete the Court's Local
Form 12, Notice of Case Specific Address. Select the correct
purpose for the new address.

Rule 5005(a)(2) permits a Court, by local rule, to permit the
filing, signing, and verifying of documents electronically. Filing
a Notice of Case Specific Address electronically deems the notice
signed.
Penalty for filing fraudulent claim: Fine of up to
$500,000 imprisonment for up to 5 years, or both. 11 U.S.C.
§§ 152 and 3571.
