The Official Rule 3002.1 Form must be attached. Supplemental documentation
to the Rule 3002.1 form may be attached.
Rule 5005(a)(2) permits a court by local rule to permit the filing,
signing, and verifying of documents electronically. Filing a
Rule 3002.1 document electronically deems the Rule 3002.1 document
signed.
Penalty for making a false statement: Fine of up to
$500,000 imprisonment for up to 5 years, or both. 18 U.S.C.
§§ 152 and 3571.
